When designing websites, I used to focus on programming the dynamic elements with MySQL and PHP, using CSS mainly to style certain elements. Later, I concentrated increasingly on the new, strict web standards to produce semantic markup, separating content from layout via CSS. Semantic markup also means using tags only according to their semantic function/content, i.e., enclosing headings in heading tags and using tables only to present tabular data, not for layout purposes.
This site was rebuild to comply to those standards.

One thing concerning terminology: layout to me is something that happens on scratch paper or in Photoshop — general decisions about the colors or fonts used, and where to place the elements on the page. Design is the process of implementing the creative masterplan laid out in the, well, layout. This happens in CSS, script languages, and HTML. It is very much concerned with the usability of a website, something that is of less importance for the layout.
